    4. Self-assessment of your capabilities in the event of an operational disruption and a crisis situation Form
      If you do not want to transmit personal information please do not complete and submit the self assessment form. This form cannot be copied-pasted, saved or printed. You will only be able to receive the results by e-mail once it has been fully completed.

      The self assessment form is intended to give you a quick portrait of your risks, vulnerabilities, weaknesses and strengths. It does constitute or replace a complete analysis of your abilities on our part does not in any way constitute advice or recommendations.

      It is intended only to assist in raising raise your awareness and make you aware of your organization’s level of preparedness and the possible impacts of an interruption of operations.We may retain the information you enter in the form for a period of 4 months (120 days). By completing this form you agree that we may communicate with you during this period.
